Player, Coach, Managers & TEAM Registration forms FOR JUne TRYOUTS

ALL PLAYERS (New and Returning) must complete:

MSYSA Player Registration form  

MSYSA Player's Rights form

BSC Parent and Player Code of Ethics form
BSC Waiver of Liability form

For MSYSA, players must also provide two 1 inch x 1 inch color full face photos and an ORIGINAL birth certificate.

ALL COACHES and MANAGERS (New and Returning) must complete:
BSC Coach Application form
BSC Coaches and Managers Code of Conduct form

MSYSA Adult Registration form

For MSYSA, coaches must provide two 1 inch x 1 inch color full face photos and a copy of your drivers license.

TEAM REGISTRATION
BBSL requires a Team Registration form for each season.
CMSA teams must complete a Team Registration form and a Team Roster form.
